The following statements identify the commitment that you, as the parent/caregiver, should display to the Avon Youth Assistance Program (AYAP) while participating in this program. This is a voluntary program and the parent/caregiver’s commitment, and involvement is an extremely important and necessary part for your student to be successful in the program.   Please read through carefully and sign if you agree with the expectations of Avon Youth Assistance Program.

  1. Communication is important and critical for AYAP to assist your child and you with your wants/needs. You agree to communicate on a regular basis with the Early Intervention Advocates. Communication can be done via txt or email.  At least once a quarter, you must meet in person.
  1. I understand that this is a voluntary program, and I will get involved by attending meetings, communicating with AYAP, and transporting my child to AYAP sponsored programming.
  1. At the end of each 9-weeks and at the end of 1st and 2nd semester, it is my responsibility, along with my student, to turn in my student’s report card to AYAP.
  1. I understand that once AYAP has financially paid for a programming opportunity, it is my responsibility to get my student to each event, activity, meeting, etc. If my student will be absent due to a sickness or emergency, I will communicate with community business along with my student’s EIA at AYAP.
  1. If I miss meetings or my student misses programming in the community on a continual basis, I understand that could restrict what programming is offered moving forward.
  1. If my student participates in the Mentorship Program through AYAP- I understand that is a one-year commitment with AYAP and their adult mentor. I agree to transport my student to a community/public place to meet with their adult mentor on a weekly basis for one hour.
  1. Mine and my student’s level of commitment can determine how long we will be in the program. I understand that if we are not following these expectations our case could potentially be closed.
  1. I understand that throughout the time in the program, the EIA will be communicating on a consistent basis with the student’s counselor, school teacher(s), and the community programming your student is involved in.
I have read this and agree to follow each of the points to the very best of my ability.
